L’équipe « Epigénétique, microenvironnement et maladies du foie » vise à mieux comprendre et cibler les maladies chroniques du foie.
Elle s’articule autour de 3 axes principaux:
- Axe 1. Hépatites chroniques B et D : persistance virale, pathogénicité et stratégies de guérison
- Axe 2. Inflammation, métabolisme et épigénétique : vers une biologie intégrative de la stéatose hépatique
- Axe 3. Soins critiques hépatiques : cibler les lésions périopératoires en chirurgie et transplantation du foie
Contexte
Les maladies du foie représentent une menace majeure pour la santé dans le monde, y compris en Europe. Les maladies chroniques du foie affectent plus de 1,5 milliard de personnes dans le monde : l’infection chronique par les virus des hépatites constitue un lourd fardeau de santé publique, avec 58 millions de personnes infectées par le VHC et 300 millions par le VHB. Par ailleurs, 10 % de la population mondiale est exposée à un risque de maladie alcoolique du foie (ALD) du fait d’une consommation excessive d’alcool. En raison de l’augmentation continue de l’obésité et du syndrome métabolique, 25 % de la population est atteinte de stéatose hépatique ou de stéatohépatite métabolique (MASLD).
Au total, les maladies chroniques du foie (CLD) sont responsables de plus de 2 millions de décès par an, liés aux complications de la cirrhose hépatique (maladie hépatique terminale – ESLD – et carcinome hépatocellulaire – HCC). Le CHC se classe au 5e rang des cancers les plus fréquents et au 3e rang des décès par cancer. La transplantation hépatique (LT) reste une intervention essentielle et salvatrice pour les patients atteints de maladie hépatique terminale ou de CHC. Toutefois, l’accès à la greffe et l’augmentation du nombre de greffons disponibles restent des défis majeurs.
Dans l’ensemble, les MCF ont un impact majeur en santé publique et en termes de coûts économiques, avec un coût individuel de santé multiplié par 4. Malgré les avancées en matière de diagnostic, la disponibilité d’un traitement curatif pour l’hépatite C, les thérapies suppressives de longue durée pour l’hépatite B chronique, les nouveaux traitements systémiques pour le CHC avancé et de meilleures options thérapeutiques pour de nombreuses causes de MCF, leur fardeau global ne cesse d’augmenter. Elles restent largement non diagnostiquées ou non traitées, notamment dans les populations migrantes.
La structuration du microenvironnement intra-hépatique, via l’interaction entre les facteurs de l’hôte (génétiques, épigénétiques, immunologiques, métaboliques) et les facteurs exogènes/environnementaux (virus, toxines, modes de vie), et son évolution, influencent à la fois la progression de la maladie et la réponse aux traitements.
Conformément à la vision stratégique de l’UMR « Pathobiologie et Thérapie des Maladies du Foie », et en s’appuyant sur l’expertise de ses membres dans les domaines fondamental, translationnel et clinique, l’équipe EpiLiv développera, en lien étroit avec l’IHU EVEREST, trois axes de recherche cohérents :

- Axe 2. Inflammation, métabolisme et épigénétique : vers une biologie intégrative de la stéatose hépatique

Axe I. Hépatites chroniques B et D : persistance virale, pathogénicité et stratégies de guérison
Dans le cadre des efforts menés pour développer de nouvelles thérapies curatives contre le VHB et le VHD, nous cherchons à mieux comprendre les mécanismes de persistance virale et de contrôle immunitaire, en nous appuyant sur des outils virologiques et immunologiques innovants.
Objectifs :
Dans le cadre des efforts plus larges de l’UMR visant à développer de nouveaux traitements contre le VHB et le VHD, EpiLiv se propose de mieux comprendre les mécanismes de persistance virale et de contrôle immunitaire, et d’identifier et valider de nouveaux biomarqueurs non-invasifs du réservoir viral intra-hépatique et des corrélats immunologiques de guérison, à travers les objectifs suivants :
- Objectif 1.1 (EpiLiv) : Ribonucleoprotéine (RNP) du VHD, mimétisme des histones et remodelage de la chromatine : rôle dans la réplication et la pathogénicité du VHD.
- Objectif 1.2 (EpiLiv) : Corrélats virologiques et immunologiques de la guérison chez les patients atteints d’hépatite B chronique.
- Objectif 1.3 (HeLiP & EpiLiv) : Rééducation du microenvironnement hépatique.
- Objectif 1.4 (HeLiP & EpiLiv) : Nouveaux biomarqueurs non-invasifs de l’activité du pool de cccDNA intra-hépatique.
Axe II. Inflammation, métabolisme et épigénétique : vers une biologie intégrative de la stéatose hépatique
Cet axe associe des études fonctionnelles visant à élucider les interactions entre inflammation, métabolisme, épigénétique, microbiotes intestinal et hépatique, avec un profilage multi-omique incluant transcriptomique en masse et à cellule unique, transcriptomique spatiale, métabolomique et lipidomique par spectrométrie de masse haute définition. Ce programme ambitieux repose sur la plateforme transcriptomique de l’équipe EpiLiv et sur sa collaboration étroite avec les plateformes métabolomiques/lipidomiques de l’Institut des Sciences Analytiques (S. Ayciriex).
Objectifs :
- Objectif 2.1 (EpiLiv) : Déterminants inflammatoires et métaboliques dans la stéatose hépatique (SLD).
- Objectif 2.2 (EpiLiv) : Lien entre métabolisme et épigénétique dans la progression vers le CHC.
- Objectif 2.3 (EpiLiv) : Rôle de l’histone méthyltransférase EZH2 dans la modulation des voies métaboliques et oncogéniques du CHC.
- Objectif 2.4 (EpiLiv & HeLiP) : Profilage clinique, immunologique et épigénétique des maladies hépatiques terminales (ESLD).
Axe III. Soins critiques hépatiques : cibler les lésions périopératoires en chirurgie et transplantation du foie
Face à la croissance rapide de la charge des MCF, de l’ESLD et du CHC, la chirurgie hépatique (résection, transplantation) est un traitement curatif majeur. Toutefois, la prise en charge chirurgicale reste complexe : en 2020 en France, 1838 patients étaient inscrits sur la liste de greffe, mais seulement 1128 ont été transplantés.
À travers les objectifs suivants, EpiLiv vise à :
i) développer de nouveaux modèles prédictifs pour la priorisation des patients sur liste d’attente ;
ii) explorer les mécanismes physiopathologiques de deux lésions post-opératoires majeures (ischémie-reperfusion et dysfonctionnement hémodynamique) ;
iii) identifier des biomarqueurs pronostiques ;
iv) évaluer des stratégies thérapeutiques pour atténuer ces lésions et améliorer les issues cliniques à court et long terme.
Objectifs :
- Objectif 3.1 (EpiLiv & HeLiP) : Nouveaux modèles prédictifs pour la priorisation des patients sur liste d’attente.
- Objectif 3.2 (EpiLiv) : Réduction des lésions d’ischémie-reperfusion en chirurgie hépatique et en transplantation.

Après avoir travaillé plusieurs années sur le virus de l’hépatite B, ses travaux visent aujourd’hui à comprendre l’évolution de la maladie hépatique à l’échelle moléculaire et de caractériser l’environnement métabolique et inflammatoire intrahépatique aux différents stades de la maladie.
Mirjam ZEISEL

Mirjam Zeisel co-dirige l’équipe EpiLiv avec Massimo Levrero.
Depuis plus de vingt ans, elle étudie les mécanismes moléculaires sous-tendant les interactions virus-hôte et les maladies hépatiques chroniques afin d’identifier de nouvelles cibles pour des stratégies préventives et thérapeutiques contre les infections par les virus de l’hépatite B, C et D ainsi que contre le carcinome hépatocellulaire.
Ses travaux actuels visent à caractériser le rôle de régulateurs épigénétiques et métaboliques dans le développement de maladies hépatiques chroniques et du carcinome hépatocellulaire, afin d’identifier de nouvelles cibles thérapeutiques.

Yasmina Chouik travaille en tant que médecin au sein du service d’hépatologie du professeur Fabien Zoulim. Elle a étudié les altérations des microARN circulants et des lymphocytes T dans la décompensation aiguë de la cirrhose lors de son doctorat à l’Institut d’hépatologie de Lyon (professeur Massimo Levrero). Actuellement, elle se concentre sur l’étude de l’inflammation systémique et intrahépatique dans les maladies hépatiques associées à un dysfonctionnement métabolique, ainsi que sur la caractérisation du microbiote intrahépatique dans les maladies hépatiques alcooliques. Yasmina Chouik dirige le projet Profil-NASH « Caractérisation approfondie de l’inflammation intrahépatique dans la stéatohépatite métabolique humaine », en collaboration avec le professeur Frank Tacke et le docteur Adrien Guillot à la Charité Universitätsmedizin de Berlin, ainsi que le projet HepMAH « Microbiote intrahépatique dans l’hépatite alcoolique ».
